Key Responsibilities

* Manage and coordinate a caseload of individuals with mental health and dementia-related needs.

* Conduct comprehensive assessments and develop person-centred care plans.

* Deliver evidence-based nursing interventions and monitor patient outcomes.

* Administer and manage medications in accordance with professional and organisational guidelines.

* Assess and manage clinical risks, promoting safe and effective care.

* Work collaboratively with multidisciplinary teams, carers, and external agencies.

* Maintain accurate clinical records and ensure compliance with relevant policies and standards.

* Provide clinical supervision, guidance, and support to junior staff and students.

* Participate in service development, quality improvement, audits, and research activities.

* Deliver health promotion, education, and recovery-focused interventions.

* Support safeguarding practices and ensure the wellbeing of vulnerable adults.

* Demonstrate leadership, professionalism, and a commitment to high-quality patient care.

Essential Requirements

* Registered Mental Health Nurse (NMC Registered).

* Degree in Nursing or equivalent qualification.

* Significant post-registration experience in mental health services.

* Experience working within community mental health settings.

* Strong assessment, risk management, communication, and care coordination skills.

* Ability to work independently and as part of a multidisciplinary team.
